You will find the new patient forms on this web site. Please have all the new patient forms pertaining to you filled out and faxed back to us as soon as possible, or you may also bring your filled out forms with you to your first visit. The more information you provide the better. Please bring a driver’s license or a picture ID.

Please have all copies of any and all lab/blood work, x-ray or any other diagnostic test (CT, MRI, PET etc..) reports over the last 12 months faxed by you or your primary care physician’s office. If necessary you can bring them with you.
You will be greeted with a very courteous, understanding, and helpful staff. We pride ourselves in taking excellent care of our patients.
On your first visit you will meet with Robert J. Gilbard, M.D. and/or Ray Hammon, D.C., N.M.D.. The Doctor’s will take a detailed history from you and review any reports. After making their recommendations as to how they feel we can best help you, one of our staff will explain any and all fees associated with each of the doctor’s recommendations. You can then make your decision about becoming a patient. We do not believe the same exact treatment for the same health issue is appropriate for every person. However, if the doctors do not feel they can help you with your health issues they will not hesitate to refer you to the appropriate specialty. All test fees include a follow-up consult at no additional charge. Your first visit will last approxamently1-2 hours.

Do not take any nutritional supplements the day of your appointment.

We work with patients from all states; therefore, you will need to be prepared to get as much accomplished as possible on your first visit to keep you from making a second trip.
-
Please do not bring children under the age of 10 to your appointment.

Most insurance companies and Medicare do not cover alternative medical procedures; therefore, we have opted out of all insurance, nor do we give you codes to file on your own.
We look forward to meeting you and seeing how we can help you with your health concerns.
IF YOU HAVE TO CANCEL YOUR APPOINTMENT A 48 HOUR NOTICE IS NEEDED SO THAT SOMEONE ELSE CAN FILL YOUR CANCELED APPOINTMENT TIME.
Thank you and Healthy Wishes.
